Report an Aircraft Accident
Federal regulations require operators to notify the NTSB immediately of aviation accidents and certain incidents. An accident is defined as an occurrence associated with the operation of an aircraft that takes place between the time any person boards the aircraft with the intention of flight and all such persons have disembarked, and in which any person suffers death or serious injury, or in which the aircraft receives substantial damage. An incident is an occurrence other than an accident that affects or could affect the safety of operations.
Contact the Response Operations Center (ROC) at 844-373-9922 to file a report. A phone call is sufficient initially, but a written follow-up may be required.

Report an ACAS/TCAS Resolution Advisory
If you are an aviation operator, Federal regulations require you to notify the NTSB immediately of ACAS/TCAS resolution advisories under certain circumstances. See
49 CFR 830 for more information. Specifically, a report is required under the following circumstances:
Airborne Collision and Avoidance System (ACAS) resolution advisories issued when an aircraft is being operated on an instrument flight rules flight plan and compliance with the advisory is necessary to avert a substantial risk of collision between two or more aircraft.
Please email the information about your ACAS/TCAS resolution advisory to
email@example.com and include the following information:
- Date and time of event (UTC);
- Flight Origin and Destination;
- Aircraft type;
- Aircraft Call Sign / Flight Number;
- Aircraft Registration Number;
- Location (approximate) of Event (with reference to easily identified location);
- Altitude at time of the event;
- Was the event reported to ATC;
- ATC Facility in contact with at time of event if known;
- Was evasive action taken;
- Crew summary of event;
- Contact e-mail address and phone number;
After evaluating a reported ACAS/TCAS RA, we will determine what level of response if any will be required. If you would like to request feedback on your report, please indicate that in your e-mail, and the assigned ATC investigator will reach out to you after they have reviewed the details of the reported event.
