A Union Pacific Railroad freight train experienced a brake malfunction while descending a grade. An air flow restriction in the brake pipe prevented the train’s air brakes from applying throughout the whole train during full service and emergency braking applications initiated from the head end. The end-of-train device lost communication with the head-of-train device, and it did not respond to the emergency braking command and bypass the restriction by releasing air from the rear of the train. The train was unable to stop and eventually collided with a stationary freight train.
