In closing, I want to recognize the NTSB staff for their excellent and thorough work bringing this report to the Board, in particular, the staff from the Office of Marine Safety and from the Office of Research and Engineering. Tom Roth-Roffy, Investigator-in-Charge, and his team did an outstanding job.

In this investigation our team identified several key issues about weather preparedness and planning and about the proper use of safety equipment, which informed the recommendations we issue today. It's also the basis for the NTSB Safety Alert for mariners highlighting good preparation and proper use of safety equipment.

An important lesson from this tragedy is the reminder that in a crisis situation everyone involved may not exhibit their best decision making or their top performance. That highlights the importance of planning and preparedness and of training and drills.

As Benjamin Franklin said so many years ago, "By failing to prepare, you are preparing to fail."
