Regional Investigations
The Office of Aviation Safety investigates almost 1700 aircraft accidents each year, most of which are general aviation accidents investigated by regional staff. While these generally do not have the public visibility of major air carrier accident investigations, these investigations can still involve safety issues important to the traveling public.

May 2012
- 5/28 - At 4:21 PM EDT, Sumerduck, VA. PA28 and unkown aircraft collided in flight 5 miles from Warrenton-Fauquier A/P (HWY). PA28 crashed in field, pilot taken to hospital, other aircraft impacted wooded area.
- 5/28 - At 4:05 PM CDT, Gulf of Mexico. Bell 206 helicopter operated by PHI, Inc., impacted the water following a main rotor impact with off-shore platform about 15 miles off coast of Louisiana.
- 5/28 - At 1:50 PM EDT, Perry, MI. North American T-6G airplane crashed into a barn near an airstrip shortly after takeoff.
- 5/28 - At 12:14 PM MDT, Fort Morgan, CO. Tackabury Air-Cam experimental airplane crashed while participating in air show activities near Fort Morgan Municipal Airport.
- 5/26 - At 5:30 AM MDT, St. George, UT. Wreckage of Cessna 172 discovered by A/P workers reporting for work.
- 5/21 - At 12PM EDT, Port Richey, FL. Pitts S1D impacted water while maneuvering.
- 5/20 - At 8:30 PM CDT, Springtown, TX. Cessna T210L crashed 3 mi. NW of Springtown.
- 5/18 - At 12:30 PM PDT, Boulder City, NV. Aero Vodochody L-39, a former East European jet trainer, crashed 2 miles east of Boulder City.
- 5/18 - At 12 PM PDT, Point Mugu, CA. Hawker Hunter MK58, crashed on approach to Point Mugu NAS.
- 5/11 - At 4:25 PM CDT, Chanute, KS. Cessna 401, IFR Riverside, KS to Council Bluffs, IA crashed.
- 5/11 - Las Vegas, NM. Beech BE-35B crashed 5 miles north of Las Vegas Municipal Airport.
- 5/10 - At 10:40 PM EDT, Sterling, PA. Mooney M20J IFR from Sterling, PA to Farmingdale, NY crashed on departure.
- 5/5 - At 12:30 PM EDT, Honesdale, PA. Cessna 177B , pilot was maneuvering for final approach when A/C collided with terrain.
- 5/3 - At 3:16 PM CDT, Crystal Lake, IL. Beech 35 crashed into a gravel pit.
