SEVERAL WITNESSES REPORTED SEEING BIG PIECES OF CRUSTED SNOW AND ICE FLYING OFF THE AIRCRAFT DURING THE TAKEOFF RUN AND CLIMB. TAKEOFF WAS MADE UP HILL CONTRARY TO PUBLISHED PROCEDURES.
