Synopsis: On October 28, 2016, about 1432 central daylight time, American Airlines flight 383, a Boeing 767-323, N345AN, had started its takeoff ground roll at Chicago O’Hare International Airport, Chicago, Illinois, when an uncontained engine failure and subsequent fire occurred. The flight crew aborted the takeoff and stopped the airplane on the runway, and the flight attendants initiated an emergency evacuation. Of the 2 flight crewmembers, 7 flight attendants, and 161 passengers on board, 1 passenger received a serious injury, and 1 flight attendant and 19 passengers received minor injuries during the evacuation. The airplane was substantially damaged from the fire. The airplane was operating under the provisions of 14 Code of Federal Regulations Part 121. Visual meteorological conditions prevailed at the time of the accident.
Recommendation: TO AMERICAN AIRLINES: For all airplanes that you operate, review existing engine fire checklists and make changes as necessary to ensure that the procedures would expeditiously address engine fires occurring on the ground without unnecessarily delaying an evacuation.

We note that you intend to adopt any changes that Boeing recommends regarding checklists for engine fires occurring on the ground. Pending completion of the recommended action, Safety Recommendation A-18-11 is classified OPEN--ACCEPTABLE RESPONSE.

Response: -From Captain Ronald Thomas, Managing Director, Flight Operations: American Airlines appreciates the opportunity to submit this response, as well as the professional and collaborative environment amongst all parties throughout this investigation. These cooperative efforts demonstrate our common commitment to aviation safety. Our Flight and Operations Safety departments along with APA Safety completed an internal assessment of our Engine Fire and Evacuation Checklists in consultation with our fleet captains and training department leaders. We have provided our input to Boeing Air Safety and we are waiting to receive their updated guidance to carriers regarding any updates to fleet checklists for engine fires occurring on the ground. Upon receipt, we will review, however our expectation is that we will adopt any Boeing-recommended changes. We remain in close coordination with Boeing and hope to have an update in the next several months. We do not plan to make any changes to these checklists for our A330, A320 family, and E190 fleets. These fleets all have a separate Engine Fire on the Ground Checklist. American Airlines values continuous assessment and incremental improvement throughout our operation. We thank the NTSB for its work and recommendations, as well as the opportunity to contribute to this investigation.

Response: The National Transportation Safety Board (NTSB) is an independent federal agency charged by Congress with investigating every civil aviation accident in the United States and significant accidents in other modes of transportation—railroad, highway, marine, and pipeline.
