Synopsis: ABOUT 10:30 A.M. ON JULY 23, 1985, IN A RURAL AREA ABOUT 8 MILES SOUTH OF KAYCEE, WYOMING, A GIRTH WELD CRACKED DURING A PIPELINE RECOATING PROJECT ON A 23-YEAR-OLD, 8-INCH DIAMETER PIPELINE OWNED BY THE CONTINENTAL PIPE LINE COMPANY. THE CRACKED GIRTH WELD ALLOWED THE RELEASE, ATOMIZATION, AND IGNITION OF AIRCRAFT TURBINE FUEL UNDER 430 POUNDS PRESSURE, KILLING ONE PERSON, BURNING SIX PERSONS, DESTROYING CONSTRUCTION EQUIPMENT, AND SHUTTING DOWN THE PIPELINE. DAMAGE WAS ESTIMATED AT MORE THAN $128,000.
Recommendation: THE NTSB RECOMMENDS THAT THE AMERICAN GAS ASSOCIATION, THE INTERSTATE NATURAL GAS ASSOCIATION OF AMERICA, AND THE AMERICAN PETROLEUM INSTITUTE: NOTIFY MEMBER COMPANIES OF THE CIRCUMSTANCES OF THE PIPELINE ACCIDENT NEAR KAYCEE, WYOMING, ON JULY 23, 1985, AND URGE THEM TO ESTABLISH OR REVIEW, AS APPROPRIATE, THEIR PROCEDURES FOR RECOATING PIPELINES, WITH PARTICULAR EMPHASIS ON THE INSPECTION OF EXPOSED GIRTH WELDS BEFORE FULLY LIFTING THE PIPE, AND TO DEVELOP PROCEDURES TO MINIMIZE THE POTENTIAL FOR A SINGLE EVENT DESTROYING ALL ONSITE COMMUNICATIONS EQUIPMENT.

Response: (SUMMARIZED) YOU INQUIRED ABOUT THE ACTION THAT API TOOK IN RESPONSE TO YOUR RECOMMENDATION TO NOTIFY MEMBERS. AS IS OUR NORMAL PRACTICE, WE SENT A COPY OF THE NTSB PIPELINE ACCIDENT REPORT TO ALL OF OUR PIPELINE COMMITTEE MEMBERS ON MARCH 6, 1986, AND ENCOURAGED THEM TO REVIEW THE RECOMMENDATIONS AND TAKE ACTION AS APPROPRIATE. AT THE SAME TIME WE INITIATED ACTION TO DEVELOP INDUSTRY GUIDELINES ON MOVING PIPELINES WHILE IN SERVICE AS I REPORTED TO YOU IN MY MAY 5, 1989 LETTER. PROGRESS IS BEING MADE ON THE GUIDELINES AND I WILL PROVIDE YOU WITH A COPY WHEN IT BECOMES AVAILABLE.

Response: OUR PIPELINE TRANSPORTATION COMMITTEE'S DESIGN AND CONSTRUCTION COMMITTEE HAS ESTABLISHED A TASK FORCE TO PREPARE IN DUSTRY GUIDELINES FOR LOWERING PIPELINES IN A MANNER CONSISTENT WITH THE NTSB'S RECOMMENDATIONS. A LITERATURE SEARCH HAS BEEN COMPLETED AND THE FIRST DRAFT OF THE NEW GUIDE SHOULD BE COMPLETED IN JUNE FOR COMMITTEE REVIEW. THE TASK FORCE CONSIDERED THE BATTELLE STUDY "GUIDELINES FOR LOWERING PIPELINES WHILE IN SERVICE, AS WELL AS OTHER METHODOLOGY. THE STRESSES EVALUATED IN EITHER LOWERING OR RAISING PIPE LINES THAT ARE IN SERVICE ARE SIMILAR AND THE PROCEDURES ESTABLISHED IN THIS GUIDE SHOULD PROVIDE FOR SAFER LOWERING OR RAISING OF PIPELINES IN SERVICE. WE WILL BE HAPPY TO PROVIDE YOU WITH A COPY OF THE NEW PUBLICATION WHEN IT BECOMES AVAILABLE. THANK YOU FOR YOUR INTEREST.
