Details
Synopsis: ON 5/14/97, THE U.S. COAST GUARD BUOY TENDER COWSLIP AND THE PANAMANIAN CONTAINER SHIP EVER GRADE COLLIDED IN THE COLUMBIA RIVER NEAR ASTORIA, OREGON. ONE PERSON WAS INJURED AS A RESULT OF THE ACCIDENT. THE EVER GRADE WAS ONLY SLIGHTLY DAMAGED IN THE COLLISION, BUT THE COWSLIP SUSTAINED SERIOUS DAMAGE TO ITS HULL AND SUPERSTRUCTURE. THE COST OF REPAIRS FOR BOTH VESSELS WAS ESTIMATED AT MORE THAN $1.2 MILLION.
Recommendation: THE NTSB RECOMMENDS THAT THE OREGON BOARD OF MARITIME PILOTS: REQUIRE ALL STATE PILOTS UNDER YOUR JURISDICTION TO COMPLETE PERIODIC BRIDGE RESOURCE MANAGEMENT TRAINING, IN ACCORDANCE WITH THE 10/5/93, RESOLUTION OF THE AMERICAN PILOTS' ASSOCIATION CONCERNING BRIDGE RESOURCE MANAGEMENT TRAINING.

Response: The Safety Board is pleased to learn that all Oregon pilots are required by OAR 856-010-0015(6) to complete a BRM-P course of instruction at least every 5 years, which was developed by various pilot associations under the auspices of the APA. Accordingly, Safety Recommendation M-99-25 is classified "Closed-Acceptable Action."

Response: Letter Mail Controlled 02/09/2001 5:29:51 PM MC# 2010103 As previously specified in our response to recommendation M-93-34, the Board currently requires its licensees to have completed a course in BRM-P. A task force appointed in 1997 conducted research and made findings and recommendations to the Board in developing a program of Continuing Professional Development for our licensees. A copy of OAR 856-010-0015 (6) is enclosed for your review. This rule details the components of the program and the frequency with which courses must be taken.
