Synopsis: ABOUT 2225, ON 6/10/95, THE PANAMANIAN PASSENGER SHIP ROYAL MAJESTY GROUNDED IN ABOUT 11 FEET OF WATER ON ROSE & CROWN SHOALS ABOUT 10 MILES EAST OF NANTUCKET ISLAND, MASSACHUSETTS. THE VESSEL, WITH 1,509 PERSONS ON BOARD, WAS EN ROUTE FROM ST. GEORGES, BERMUDA, TO BOSTON, MASSACHUSETTS, WHEN THE ACCIDENT OCCURRED. THE GROUNDING OCCURRED IN AN AREA OF HARD SAND THAT CAUSED THE VESSEL TO BE TRIMMED ABOUT 10 FEET BY THE STERN & TO ASSUME A 3.5 DEGREES STARBOARD LIST. THE VESSEL'S ENGINEERING & HABITABILITY SYSTEMS, HOWEVER, REMAINED FULL OPERATIONAL.
Recommendation: THE NTSB RECOMMENDS THAT THE USCG: IMMEDIATELY RECOMMEND THAT THE INTERNATIONAL MARITIME ORGANIZATION URGE ITS ADMINISTRATIONS TO ADVISE MARITIME VESSEL OPERATORS OF THE CIRCUMSTANCES OF THE ROYAL MAJESTY GROUNDING & TO ENCOURAGE THE OPERATORS TO REVIEW THE DESIGN OF THEIR INTEGRATED BRIDGE SYSTEMS TO IDENTIFY POTENTIAL SYSTEMS & OPERATIONAL FAILURE MODES THAT MIGHT RESULT IN UNDTECTED CHANGES TO THE AUTOPILOT FUNCTION & DEVELOP MODIFICATIONS AS REQUIRED. Response: THE BOARD IS DISAPPOINTED THAT THIS RECOMMENDATION COULD NOT BE OFFICIALLY ENTERED INTO IMO'S SUBNAV-41 PROCEEDINGS DUE TO PUBLICATION APPROVAL & TIME RULES, WE ARE NONETHELESS PLEASED THAT THE USCG DISTRIBUTED NUMEROUS COPIES OF THE RECOMMENDATION & THAT IT WAS DISCUSSED IN SEVERAL OF THE TECHNICAL WORKING GROUP SESSIONS. BECAUSE THE USCG WAS ABLE TO DISSEMINATE THIS INFO TO THE INTERNATIONAL COMMUNITY, THEREBY SATISFYING THE INTENT OF THE RECOMMENDATION, M-95-26 HAS BEEN CLASSIFIED "CLOSED--ACCEPTABLE ALTERNATE ACTION."

Response: WE CONCUR WITH THIS RECOMMENDATION. THE INTERNATIONAL MARITIME ORGANIZATION'S SUBCOMMITTEE ON SAFETY OF NAVIGATION WILL BE MEETING LONDON, SEPTEMBER 18-22, 1995. THAT WILL BE THE SOONEST & MOST APPROPRIATE OPPORTUNITY TO ADVISE MEMBER GOVERNMENTS & NONGOVERNMENTAL OBSERVERS OF THE CIRCUMSTANCES OF THE ROYAL MAJESTY CASUALTY. THE U.S. DELEGATION WILL PROVIDE COPIES OF THE NTSB RECOMMENDATION LETTER TO DELEGATIONS AT THE SESSION, & WILL URGE THEM TO COMPLY WITH THE RECOMMENDATIONS.
