Synopsis: ON 2/16/95, AT 2027 EASTERN STANDARD TIME, A DOUGLAS DC-8-63, OPERATED BY AIR TRANSPORT INTERNATIONAL (ATI), CRASHED AS THE FLIGHTCREW WAS ATTEMPTING TO MAKE A 3-ENGINE TAKEOFF FROM RUNWAY 01 LEFT AT KANSAS CITY INTERNATIONAL AIRPORT (MCI), KANSAS CITY, MISSOURI.
Recommendation: THE NTSB RECOMMENDS THAT THE FAA: LIMIT OPERATIONS OF ENGINE-OUT FERRY FLIGHTS TO TRAINING, FLIGHT TEST, OR STANDARDIZATION FLIGHTCREWS THAT HAVE BEEN SPECIFICALLY TRAINED IN ENGINE-OUT PROCEDURES.

Response: THE BOARD NOTES THAT THE FAA HAS ISSUED FLIGHT STANDARDS INFO BULLETIN 95-16A, DIRECTING PRINCIPAL OPERATIONS INSPECTORS TO INFORM THEIR RESPECTIVE OPERATORS TO TAKE ADDITIONAL MEASURES TO ENSURE: (1) THAT AIRCRAFT MANUAL REQUIREMENTS FOR ENGINE-OUT FERRY FLIGHTS ARE CLEAR & UNAMBIGOUS; (2) THAT FLIGHTCREW TRAINING SEGMENTS ARE CLEARLY OUTLINED FOR ENGINE-OUT OPERATION; & (3) THAT ENGINE-OUT FERRY FLIGHTS ARE FLOW ONLY BY FLIGHTCREW MEMBERS SPECIFICALLY RECOMMENDED BY THE CHIEF PILOT & APPROVED BY THE POI THROUGH A LETTER OF AUTHORIZATION. BECAUSE THIS BULLETIN SATISFIES THE INTENT OF THE RECOMMENDATION, THE BOARD CLASSIFIES A-95-39 "CLOSED--ACCEPTABLE ACTION."

Response: THE FAA AGREES WITH THIS RECOMMENDATION & ISSUED A FLIGHT STANDARDS INFO BULLETIN. THIS BULLETIN DIRECTS PRINCIPAL OPERATIONS INSPECTORS TO INFORM THEIR RESPECTIVE OPERATORS TO TAKE ADDITONAL MEASURES TO ENSURE: (1) THAT AIRCRAFT MANUAL REQUIREMENTS FOR ENGINE-OUT FERRY FLIGHTS ARE CLEAR; (2) THAT FLIGHTCREW TRAINING SEGMENTS ARE CLEARLY OUTLINED FOR ENGINE-OUT OPERATIONS; & (3) OPERATORS USE ONLY FLIGHTCREWS SPECIFICALLY TRAINED & CERTIFIED FOR ENGINE-OUT OPERATIONS. I WILL PROVIDE THE BOARD WITH A COPY OF THE BULLETIN AS SOON AS IT IS ISSUED.
