Synopsis: ON OCTOBER 26, 1993, ABOUT 1552, N82, A BEECH SUPER KING AIR 300/F (BE-300/F, OWNED BY THE FEDERAL AVIATION ADMINISTRATION (FAA) AND OPERATED BY THE ATLANTIC CITY (ACY), NEW JERSEY, FLIGHT INSPECTION AREA OFFICE (FIAO), WAS DESTROYED WHEN IT CRASHED INTO MOUNTAINOUS TERRAIN NEAR FRONT ROYAL, VIRGINIA. THE THREE FLIGHT CREWMEMERS ABOARD RECEIVED FATAL INJURIES.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: FOR THE PURPOSE OF MISSION MANAGEMENT, ESTABLISH FORMAL MISSION BRIEFING AND DEBRIEFING REQUIREMENTS FOR FAA FLYING OPERATIONS THAT INVOLVE AN OPERATIONS SUPERVISOR, THE PIC, AND ALL CREWMEMBERS.

Response: A-94-090 ASKED THE FAA TO ESTABLISH, FOR THE PURPOSE OF MISSION MANAGEMENT, FORMAL MISSION BRIEFING & DEBRIEFING REQUIREMENTS FOR FAA FLYING OPERATIONS THAT INVOLVE AN OPERATIONS SUPERVISOR, THE PILOT-IN COMMAND (PIC), & ALL CREWMEMBERS. ON 4/19/95, TECHNICAL INSPECTION MANUAL 4040.5OA, "FLIGHT INSPECTION OPERATIONS MANUAL," BECAME EFFECTIVE & PRESCRIBES THE FORMAL MISSION BRIEFING & DEBRIEFING REQUIREMENTS FOR THE FLIGHT INSPECTION PROGRAM. A COPY OF THE MANUAL WAS PROVIDED. MISSION BRIEFING DEBRIEFING FOR ALL OTHER AIRCRAFT PROGRAM PARTICIPANTS ARE PRESCRIBED IN FAA ORDER 4040.9D, CHANGE 4, CHAPTER 6, PARAGRAPH 609. THIS CHANGE REQUIRES THAT THE OPERATIONS SUPERVISOR, PIC & ALL CREWMEMBERS BE INVOLVED IN THE BRIEFINGS. BECAUSE MISSION BRIEFINGS ARE NOW A PART OF ALL FAA AIRCRAFT PROGRAM FLYING OPERATIONS, THE BOARD CLASSIFIES A-94-90 "CLOSED--ACCEPTABLE ACTION."

MISSION BRIFEFING & DEBRIEFING FOR ALL OTHER AIRCRAFT PROGRAM PARTICIPANTS ARE PRESCRIBED IN FAA ORDER 4040.9D, CHANGE 4, CHAPTER 6, PARAGRAPH 609. THIS CHANGE REQUIRES THAT THE OPERATIONS SUPERVISOR, PILOT-IN-COMMAND, & ALL CREWMEMBERS BE INVOLVED IN THE BRIEFINGS. I CONSIDER THE FAA'S ACTION TO BE COMPLETED ON THIS RECOMMENDATION.

THE SCOPE OF THE RESPONSE APPEARS TO BE LIMITED TO AIRPLANES OPERATED UNDER THE PROVISIONS OF 14 CFR PART135 & THE FLIGHT INSPECTION OPERATION MANUAL. THEREFORE, THE BOARD AWAITS AN ADDITIONAL RESPONSE THAT DEALS WITH FORMAL MISSION BRIEFING & DEBRIEFING REQUIREMENTS FOR ALL AIRCRAFT PROGRAM PARTICIPANTS & ALL AIRCRAFT OPERATED (I.E., OWNED, LEASED, OR RENTED) BY THE FAA. Response: PREVIOUSLY, THE FAA STATED THAT IS WOULD REVISE THE FLIGHT INSPECTION OPERATIONS MANUAL TO MEET THE INTENT OF THIS RECOMMENDATION. A DRAFT VERSION (TI 4040.50A) RECENTLY ACCEPTED BY THE DALLAS/FORTH WORTH CERTIFICATE MANAGEMENT OFFICE WILL BE EFFECTIVE CONCURRENTLY WITH CERTIFICATION OF THE FLIGHT INSPECTION PROGRAM. THIS MANUAL IS INTENDED FOR USE BY ALL PERSONNEL WHOSE DUTIES INVOLVE THE CONDUCT & SUPPORT OF FLIGHT INSPECTION AIRCRAFT OPERATIONS. THE DRAFT MANUAL IS SCHEDULED FOR PUBLICATION IN MARCH 1995. I WILL KEEP THE BOARD APPRISED OF THE FAA'S PROGRESS ON THIS RECOMMENDATION.

SPECIFIC PROCEDURES & ITEMS OF CONTENT WILL BE FURTHER DELINEATED TO ENSURE THAT ALL FLIGHT CREWMEMBERS ARE FAMILIAR WITH ALL ASPECTS OF THE PLANNED MISSION AS THEY OCCUR. THIS REVISION WILL BE INCLUDED IN THE AUGUST 1994 CHANGE TO THE FLIGHT INSPECTION OPERATIONS MANUAL.
