Synopsis: ON OCTOBER 26, 1993, ABOUT 1552, N82, A BEECH SUPER KING AIR 300/F (BE-300/F, OWNED BY THE FEDERAL AVIATION ADMINISTRATION (FAA) AND OPERATED BY THE ATLANTIC CITY (ACY), NEW JERSEY, FLIGHT INSPECTION AREA OFFICE (FIAO), WAS DESTROYED WHEN IT CRASHED INTO MOUNTAINOUS TERRAIN NEAR FRONT ROYAL, VIRGINIA. THE THREE FLIGHT CREWMEMERS ABOARD RECEIVED FATAL INJURIES.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: DEVELOP AND IMPLEMENT A PROGRAM GUARANTEEING THAT PERSONNEL WHO BRING SAFETY-RELATED CONCERNS TO THE ATTENTION OF MANAGMENT CAN DO SO WITHOUT FEAR OF RETRIBUTION, AND WITH ASSURANCE THAT SUCH CONCERNS WILL BE ADDRESSED THOROUGHLY AND IMPARTIALLY. Response: THE BOARD NOTES THAT THE FAA HAS INITIATED SEVERAL PROCESSES, INCLUDING A SAFETY "HOT LINE" TO BRING SAFETY-RELATED CONCERNS TO THE ATTENTION OF MANAGEMENT WITHOUT HAVING EMPLOYEES FEAR RETRIBUTION OR REPRISAL. THEREFORE, THE BOARD CLASSIFIES A-94-88 "CLOSED--ACCEPTABLE ACTION."

Response: THE FAA HAS INITIATED SEVERAL PROCESSES TO BRING SAFETY-RELATED CONCERNS TO THE ATTENTION OF MANAGEMENT WITHOUT EMPLOYEE FEAR OF RETRIBUTION OR REPRISAL. A SAFETY "HOT LINE" WILL BE INSTALLED BY AUGUST 1994 IN THE OFFICE OF THE FAA SENIOR FLIGHT SAFETY OFFICER. ANONYMITY WILL BE ENSURED FOR ALL EMPLOYEES WISHING TO USE THE "HOT LINE." FAA FORM 4040-8 SAFETY IMPROVEMENT/HAZARD DETECTION REPORT, WAS DISTRIBUTED TO HEADQUARTERS, REGIONAL, & CENTER EMPLOYEES FOR USE IN REPORTING SAFETY CONCERNS. NOTICE 4040.XX, FAA FLIGHT SAFETY PROGRAM, WILL BE DISTRIBUTED TO ALL FLIGHT PROGRAM PARTICIPANTS IN AUGUST 1994. THIS NOTICE IDENTIFIES PROCEDURES FOR REPORTING AERONAUTICAL OCCURRENCES & SAFETY HAZARDS. ADDITIONALLY, A MANAGEMENT PHILOSOPHY STATEMENT CONCERNING SAFETY OF FLIGHT ISSUES HAS BEEN COMMUNICATED TO APPROPRIATE EMPLOYEES & IS POSTED THROUGHOUT THE WORKPLACE. THIS PHILOSOPHY STATEMENT EMPHASIZES SAFETY AS THE HIGHEST PRIORITY WITH RESPECT TO MISSION ACCOMPLISHMENT.
