Synopsis: ON OCTOBER 26, 1993, ABOUT 1552, N82, A BEECH SUPER KING AIR 300/F (BE-300/F, OWNED BY THE FEDERAL AVIATION ADMINISTRATION (FAA) AND OPERATED BY THE ATLANTIC CITY (ACY), NEW JERSEY, FLIGHT INSPECTION AREA OFFICE (FIAO), WAS DESTROYED WHEN IT CRASHED INTO MOUNTAINOUS TERRAIN NEAR FRONT ROYAL, VIRGINIA. THE THREE FLIGHT CREWMEMERS ABOARD RECEIVED FATAL INJURIES.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: IMPROVE CRITERIA TO SPECIFY THE OPERATIONAL AND MAINTENANCE RELATED INCIDENTS THAT ARE REQUIRED TO BE REPORTED TO A CENTRAL AVN AUTHORITY; AND IMPLEMENT PROCEDURES TO VERIFY THAT ALL INCIDENTS MEETING SUCH CRITERIA ARE BEING REPORTED AS REQUIRED. Response: THE BOARD NOTES THAT THE FAA IS DEVELOPING REPORTING PROCEDURES FOR OPERATIONAL INCIDENTS TO BE CONTAINED IN TI 4040.50, "FLIGHT INSPECTION OPERATIONS MANUAL," & THAT MAINTENANCE-RELATED INCIDENT REPORTING PROCEDURES ARE CONTAINED IN TI 4100.24, CHAPTER VI, "GENERAL MAINTENANCE MANUAL." THEREFORE, THE BOARD CLASSIFIES A-94-87 'CLOSED ACCEPTABLE ACTION."

REPORTING PROCEDURES FOR OPERATIONAL-RELATED INCIDENTS ARE BEING DEVELOPED & WILL BE CONTAINED IN TI 4040.50 FLIGHT INSPECTION OPERATION MANUAL. THIS MANUAL WILL BE EFFECTIVE 7/31/94. MAINTENANCE-RELATLED INCIDENT REPORTING PROCEDURES HAVE BEEN DEVELOPED & ARE CONTAINED IN TI 4200.24, CHAPTER VI, GENERAL MAINTENANCE MANUAL.
