Synopsis: ON NOVEMBER 21, 1991, AS THE RESULT OF THE INVESTIGATION OF TWO COMMUTER AIRLINE ACCIDENTS, THE NATIONAL TRANSPORTATION SAFETY BOARD ADOPTED SAFTY RECOMMENDATION A-91-122. ALSO AS THE RESULT OF THE INVESTIGATION OF THE SAME TWO ACCIDENTS, ON JULY 22, 1992, THE SAFETY BOARD ADOPTED RECOMMENDATION RECOMMENDATIONS A-92-59, -60, AND -61. THE FAA AREED WITH SAFETY RECOMMENDATION A-91-122 IN A LETTER TO SAFETY BOARD, DATED JANUARY 31, 1992, ADDING THAT AN ACOB WAS BEING PREPARED TO ADDRESS THE SUBJECT. ON APRIL 10, 1992, THE SAFETY BOARD CLASSIFIED A-91-122 AS "OPEN--ACCEPTABLE RESPONSE," PENDING THE ISSUANCE OF THE ACOB. ON OCTOBER 16, 1992, THE FAA RESPONDED THAT IT AGREED WITH SAFETY RECOMMENDATIONS A-92-59,-60, AND -61 AND THAT IT WOULD HANDLE THE ISSUES IN THE ACOB, WHICH WAS BEING DRAFTED. ON APRIL 16, 1993, THE SAFETY BOARD CLASSIFIED THESE RECOMMENDATIONS, "OPEN--ACCEPTABLE RESPONSE."
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: TAKE THE APPROPRIATE ACTIONS TO VERIFY THAT ACOBS ISSUED IN THE PAST FEW YEARS HAVE BEEN IMPLEMENTED AS INTENDED.

Response: THE BOARD NOTES THAT THE FAA HAS ISSUED NOTICE 8430.410, "VERIFICATION OF ACTIONS: AIR CARRIER OPERATIONS BULLETINS," THAT DIRECTS ALL FLIGHT STANDARDS FIELD MANAGERS TO VERIFY THAT THE ACTIONS CONTAINED IN ACOBS ISSUED BETWEEN 1/1/92, & 12/31/94, HAVE BEEN ACCOMPLISHED. ADDITIONALLY, SPECIFIC DIRECTIONS WERE PROVIDED TO ENSURE THAT ACOB 8-93-4, "FLIGHT IN POTENTIAL ICING CONDITIONS & THE AVOIDANCE, RECOGNITION, & RESPONSE TO TAILPLANE ICE," HAS BEEN ACCOMPLISHED. THE BOARD HAS REVIEWED THE FAA NOTICE & FINDS THAT IT MEET THE INTENT OF THESE RECOMMENDATIONS A-94-071 & -72 HAVE BEEN CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

THIS NOTICE DIRECTS ALL FLIGHT STANDARDS FIELD OFFICE MANAGERS TO VERIFY THAT THE ACTIONS CONTAINED IN ALL ACOB'S ISSUED BETWEEN 1/1/92, & 12/31/93, HAVE BEEN ACCOMPLISHED. THIS ACTION INCLUDES VERIFYING THAT THE SAFETY RELATED ACTIONS CONTAINED IN ACOB 8-93-4, FLIGHT IN POTENTIAL ICING CONDITIONS & THE AVOIDANCE, RECOGNITION, & RESPONSE TO TAILPLANE ICE, HAVE BEEN ACCOMPLISHED. I CONSIDER THE FAA'S ACTION TO BE COMPLETED ON THESE RECOMMENDATIONS.
