Synopsis: ON SUNDAY, MARCH 22, 1992, ABOUT 2135 EASTERN STANDARD TIME, A FOKKER 28-4000 (F-28), N485US, OPERATING AS USAIR FLIGHT 405, CRASHED DURING AN ATTEMPTED TAKEOFF FROM RUNWAY 13 AT LAGUARDIA AIRPORT, FLUSHING, NEW YORK. THE AIRPLANE WAS OPERATING UNDER TITLE 14, CODE OF FEDERAL REGULATIONS (CFR), PART 121, AS A SCHEDULED PASSENGER FLIGHT FROM JACKSONVILLE, FLORIDA, TO CLEVELAND, OHIO, WITH A STOPOVER AT LAGUARDIA AIRPORT. THERE WERE 47 PASSENGERS, 2 FLIGHTCREW MEMBERS AND 2 CABINCREW MEMBERS ON BOARD. THE CAPTAIN, ONE OF THE CABINCREW MEMBERS, AND 25 PASSENGERS RECEIVED FATAL INJURIES. IMPACT FORCES AND THE SUBSEQUENT FIRE DESTROYED THE AIRPLANE.
Recommendation: THE NTSB RECOMMENDS THAT THE PORT AUTHORITY: EXPEDITE THE REPLACEMENT OF THE EMERGENCY TELEPHONE SYSTEM BETWEEN THE AIR TRAFFIC CONTROL TOWER AND ARFF UNITS AT LAGUARDIA AIRPORT.

THE BOARD BELIEVES THE RESPONSE TO ITS RECOMMENDATIONS IS VERY TIMELY, CONSIDERING THE SCOPE OF THE TWO PROJECTS. PENDING COMPLETION OF THE PROJECTS, THE BOARD CLASSIFIES BOTH RECOMMENDATIONS "OPEN--ACCEPTABLE RESPONSE."

Response: WITH RESPECT TO PUMP STATION #3, THE WORK REQUIRED TO SUBMERGE THE PUMP AS WELL AS RELOCATE THE ASSOCIATED ELECTRICAL EQUIPMENT IS BEING FINALIZED & WE EXPECT COMPLETION BY 10/1/94. AS YOU ARE AWARE FROM A PREVIOUS CORRESPONDENCE DATE 10/15/93, THE SUBMERSIBLE SYSTEMS; THEREFORE, REDESIGN OF THESE ELECTRICAL SYSTEMS WAS NECESSARY. THE CONTRACTS FOR PUMPS #1 AND #2 WERE AWARDED IN MARCH 1994 & WE EXPECT THIS WORK TO BE COMPLETED BY MARCH 1995. WITH RESPECT TO THE EMERGENCY CONFERENCE LINE TELEPHONE SYSTEM, THIS HAS BEEN BROKEN DOWN INTO TWO PHASES. DURING PHASE 1 OF THE INSTALLATION OF THE EMERGENCY ALERT NOTIFICATION SYSTEM, THE OLD SYSTEM WAS REPLACED WITH AN UPDATED STATE-OF-THE-ART COMMUNICATION SYSTEM CONNECTING THE FAA CONTROL TOWER AT LAGUARDIA AIRPORT TO VARIOUS PORT AUTHORITY UNITS & NEW YORK CITY EMERGENCY RESPONSE AGENCIES. THIS SYSTEM ALSO INCLUDES A BACKUP MASTER STATION UNIT WHICH CAN BE UTILIZED IF THE PRIMARY UNIT IN THE CONTROL TOWER SHOULD BECOME INOPERATIVE. PHASE 1 OF THE SYSTEM WAS PUT INTO OPERATION ON 2/25/94. ONCE PHASE 2 OF THE SYSTEM IS COMPLETED, THE SYSTEM WILL HAVE THE CAPABILITY OF PERFORM SELF-MONITORING & FAILURE REPORTING, REPEAT AN EMERGENCY ANNOUNCEMENT THREE TIMES, MAINTAIN A PRINTED LOG/RECORD OF EMERGENCY NOTIFICATION/LOCATION PICKUP RESPONSE, AS WELL AS DATE & TIME OF RECORD. COMPLETION OF PHASE 2 INSTALLATION IS SCHEDULED FOR ON OR ABOUT 9/1/94. THE FORMER SYSTEM WILL REMAIN CONNECTED FOR USE, IF NECESSARY UNTIL PHASE 2 IS COMPLETED TO THE PORT AUTHORITY'S SATISFACTION.

UNTIL THE NEW SYSTEM IS INSTALLED, OUR CURRENT SYSTEM CONTINUES TO BE FULLY OPERATIONAL AND PROVIDE SATISFACTORY EMERGENCY COMMUNICATIONS.
