Synopsis: ON DECEMBER 3, 1990, AT 1354 EASTERN STANDARD TIME, NORTHWEST AIRLINES (NWA) FLIGHT 1482, A MCDONNELL DOUGLAS DC-9, AND NORTHWEST AIRLINES FLIGHT 299, A BOEING 727 (B-727), COLLIDED NEAR THE INTERSECTION OF RUNWAYS 09/27 AND 03C-21C AT DETROIT METROPOLITAN/WAYNE COUNTY AIRPORT (DTW), DETROIT MICHIGAN. THE DC-9 WAS TO BE A REGULARLY SCHEDULED PASSENGER FLIGHT TO PITTSBURGH, PENNSYLVANIA, AND THE B-727 WAS TO BE A REGULARLY SCHEDULED PASSENGER FLIGHT TO MEMPHIS, TENNESSEE. BOTH AIRPLANES WERE OPERATING UNDER FEDERAL AVIATION REGULATIONS (FAR) PART 121 AND INSTRUMENT METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME AT DTW. THE B-727 WAS ON ITS TAKEOFF ROLL ON RUNWAY 3C AT THE TIME OF THE COLLISION, AND THE DC-9 HAD TAXIED ONTO THE RUNWAY JUST PRIOR TO THE ACCIDENT. THE B-727 WAS SUBSTANTIALLY DAMAGED, AND THE DC-9 WAS DESTROYED DURING THE COLLISION AND SUBSEQUENT FIRE. OF THE 40 PASSENGERS AND 4 CREWMEMBERS ABOARD THE DC-9, 7 PASSENGERS AND 1 FLIGHT ATTENDANT RECEIVED FATAL INJURIES. NONE OF THE 146 PASSENGERS AND 8 CREWMEMBERS ON THE B-727 WERE INJURED.
Recommendation: THE NTSB RECOMMENDS THAT THE DETROIT METROPOLITAN/WAYNE COUNTY AIRPORT: IMPLEMENT A PROGRAM TO PROVIDE FOR THE PROMPT REPAINTING OF FADED TAXIWAY AND RUNWAY MARKINGS WHEN THEY ARE SEEN DURING DAILY AIRPORT INSPECTIONS, RATHER THAN WAITING FOR A SET SCHEDULE FOR OVERALL AIRPORT RESTRIPING.

Response: THE BOARD NOTES THAT THE DETROIT METROPOLITAN WAYNE COUNTY AIRPORT HAS INSTALLED SEMIFLUSH RUNWAY EDGE LIGHTS IN ACCORDANCE WITH AC-150/5340-24 AT THE RUNWAY 3C/21C AND 9L/27R INTERSECTION; HAS IMPLEMENTED A PROGRAM THAT, WEATHER PERMITTING, ALLOWS IT TO REPAINT FADED AIRFIELD MARKINGS YEAR ROUND, IN ADDITION TO THE ANNUAL REPAINTING OF THE AIRFIELD; AND HAS A LIGHTING PROJECT UNDER DESIGN TO REPLACE ELECTRICAL WIRING AND UPGRADE DISTRIBUTION ARRANGEMENT THAT MEETS THE MOST RECENT FAA STANDARDS. THE AIRPORT IS NOW USING PAINT CONTAINING REFLECTIVE BEADS ON HOLDLINES AND TAXIWAY CENTERLINES, AND HAS IMPLEMENTED A PROGRAM TO APPLY A BLACK BACKGROUND/BORDER FOR ALL HOLDLINES AND TAXIWAY CENTERLINES. THE BOARD APPRECIATES THE EFFORTS TAKEN TO ADDRESS THESE RECOMMENDATIONS AND BELIEVES THAT THESE ACTIONS SHOULD REDUCE THE RUNWAY INCURSION HAZARD. ACCORDINGLY, RECOMMENDATION A-91-67 AND -68 ARE CLASSIFIED "CLOSED--ACCEPTABLE ACTION."

Response: PURSUANT TO THE BOARD RECOMMENDATIONS A-91-67 AND A-91-68, DETROIT METROPOLITAN WAYNE COUNTY AIRPORT HAS TAKEN THE FOLLOWING ACTION. A-91-67: SEMIFLUSH RUNWAY EDGE LIGHTS WERE INSTALLED IN ACCORDANCE WITH ADVISORY CIRCULAR 150/5340-24 AT THE RUNWAY 3C/21C AND 9L/27R INTERSECTION. CONSTRUCTION WAS COMPLETED IN SEPTEMBER, 1991. A-91-68: WE HAVE IMPLEMENTED A PROGRAM THAT, WEATHER PERMITTING, ALLOWS US TO REPAINT FADED AIRFIELD MARKINGS ALL YEAR ROUND, IN ADDITION TO THE ANNUAL REPAINTING OF THE AIRFIELD. IN ADDITION TO THE BOARD RECOMMENDATIONS CURRENTLY UNDER DESIGN IS A LIGHTING PROJECT WITH OBJECTIVES OF REPLACING ELECTRICAL WIRING AND UPGRADING DISTRIBUTION ARRANGEMENT AS WELL AS MEETING THE MOST RECENT FAA STANDARDS AND REQUIREMENTS FOR LIGHTING SPACING, TYPES, SIGNAGE LOCATIONS, NOTATIONS AND SIGNAGE UPGRADING. RHEOSTAT CONTROLS IN THE FAA CONTROL TOWER AIRFIELD LIGHTING PANEL WERE REPLACED WITH SWITHCES EQUIPPED WITH STOPS IN ACCORDANCE WITH AC 15/5345-3D. ALSO, WE HAVE REVISED OUR POLICY TO NOW USE PAINT CONTAINING REFLECTIVE BEADS ON HOLDLINES AND TAXIWAY CENTERLINES. FURTHER DETROIT METROPOLITAN WAYNE COUNTY AIRPORT HAS IMPLEMENTED A PROGRAM TO APPLY A BLACK BACKGROUND/BORDER FOR ALL HOLDLINES AND TAXIWAY CENTERLINES TO FURTHER ENHANCE VISIBILITY.
