Details
Synopsis: ON DECEMBER 3, 1990, AT 1354 EST, NORTHWEST AIRLINES FLIGHT 299, A BOEING B-727, AND NORTHWEST FLIGHT 1482, A MCDONNELL DOUGLAS DC-9, COLLIDED NEAR THE INTERSECTION OF RUNWAY 03C/ 21C AND 09/27 AT DETROIT METROPOLITAN/ WAYNE COUNTY AIRPORT, ROMULUS, MICHIGAN. THE B-727 WAS TAKING OFF FROM RUNWAY 03C, AND THE DC-9 HAD TAXIED ONTO THAT RUNWAY JUST PRIOR TO THE ACCIDENT. VISIBILITY WAS REPORTED TO HAVE BEEN 1/4 MILE IN FOG AT THE TIME. ONE FLIGHT ATTENDANT AND SEVEN PASSENGERS ONBOARD THE DC-9 WERE KILLED; THERE WERE NO INJURIES TO THE PERSONS ONBOARD THE B-727.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: IMMEDIATELY REQUIRE THAT THE SAFETY BOARD'S FINDINGS THUS FAR REGARDING THE DC-9 MANUAL TAILCONE RELEASE BE PROVIDED TO OPERATORS OF DC-9/MD-80 AIRPLANES AND THEIR CREWMEMBERS. (URGENT)

Response: The Safety Board notes that on December 20, 1990, the FAA issued Notice 8300.86, McDonnell Douglas DC-9 Tail Cone Emergency Exit Maintenance Program, which is applicable to the DC-9 series aircraft including the MD-80. This notice meets the intent of this safety recommendation, and the Safety Board classifies Safety Recommendation A-91-5 as "Closed--Acceptable Action."

Response: ON 12/20/90, THE FAA ISSUED NOTICE 8300.86, MCDONNELL DOUGLAS DC-9 TAILCONE EMERGENCY EXIT MAINTENANCE PROGRAMS, TO REQUIRE A FLEETWIDE INSPECTION OF TAILCONE EMERGENCY EXIT MAINTENANCE PROGRAMS. THIS NOTICE IS APPLICABLE TO THE DC-9 SERIES AIRCRAFT, INCLUDING THE MD-80. THIS NOTICE, WHICH CONTAINS A SUMMARY OF THE SAFETY BOARD'S FINDINGS, DIRECTS PRINCIPAL MAINTENANCE/OPERATIONS INSPECTORS TO NOTIFY IMMEDIATELY ALL DC-9 OPERATORS OF THE POTENTIAL FOR HANDLE FAILURES AND RESULTANT EGRESS SYSTEM FAILURE IF AN EXCESSIVE SIDE LOAD IS PLACED ON THE HANDLE. THIS INSPECTION INCLUDES REVIEWING THE PROCEDURES FOR THE RELEASE HANDLE/LATCH AND CABLE MECHANISM, CABLE TENSION RELEASE MEASUREMENTS, PLACARDING, AND RIGGING. THE NOTICE DIRECTS CERTIFICATE HOLDING DISTRICT OFFICES TO REVIEW THE OPERATOR'S MAINTENANCE PROGRAM TO ENSURE THAT PROCEDURES ARE INCLUDED FOR SYSTEM CLEANING, CORROSION CONTROL, AND APPROPRIATE LUBRICATION AND TO VERIFY THAT INSPECTION INTERVALS AND PROCEDURES FOR SYSTEM DEPLOYMENT CHECKS ARE INCLUDED AS PART OF THE OVERALL MAINTENANCE PROGRAM.
