Details
Synopsis: ON DECEMBER 3, 1990, AT 1354 EST, NORTHWEST AIRLINES FLIGHT 299, A BOEING B-727, AND NORTHWEST FLIGHT 1482, A MCDONNELL DOUGLAS DC-9, COLLIDED NEAR THE INTERSECTION OF RUNWAY 03C/ 21C AND 09/27 AT DETROIT METROPOLITAN/ WAYNE COUNTY AIRPORT, ROMULUS, MICHIGAN. THE B-727 WAS TAKING OFF FROM RUNWAY 03C, AND THE DC-9 HAD TAXIED ONTO THAT RUNWAY JUST PRIOR TO THE ACCIDENT. VISIBILITY WAS REPORTED TO HAVE BEEN 1/4 MILE IN FOG AT THE TIME. ONE FLIGHT ATTENDANT AND SEVEN PASSENGERS ONBOARD THE DC-9 WERE KILLED; THERE WERE NO INJURIES TO THE PERSONS ONBOARD THE B-727.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: IMMEDIATELY REQUIRE A FLEET-WIDE INSPECTION OF ALL DC-9 TAILCONE ASSEMBLIES, AS OUTLINED IN THE DC-9 MAINTENANCE MANUAL "TAILCONE-MAINTENANCE PRACTICES" "53-50-3" "CODE 1" PAGES 201 THROUGH 208A AND THE MD-80 MAINTENANCE MANUAL "TAILCONE-MAINTENANCE PRACTICES" "53-50-3" PAGES 201 TO 221; REQUIRE DETAILED VISUAL EXAMINATION OF THE INTERIOR AND EXTERIOR TAILCONE RELEASE HANDLES FOR BROKEN OR CRACKED SHAFTS AND FOR DAMAGE FROM CONTACT WITH THE LOCK CABLE BALL END FITTING (DOUGLAS PART NUMBER 492740); AND REQUIRE THAT DAMAGE HANDLES BE REPAIRED OR REPLACED. Response: The Safety Board notes that on January 10, 1991, the FAA issued an airworthiness directive (AD) (Docket No. 91-NM-04-AD, Admendment 39-6867) to require repetitive inspections and functional checks of the DC-9 tailcone release system for proper operation. The AD requires inspection of the emergency exit system in accordance with McDonnell Douglas Corporation's Alert Service Bulletins A53-243 (emergency exit system inspection) and A53-242 (tailcone handle inspection). Based on the above information, Safety Recommendation A-91-003 is classified as "Closed--Acceptable Action."

Response: On March 1, 1991, the Federal Aviation Administration (FAA) issued its final rule (Docket No. 91-NM-21-AD) to adopt an airworthiness directive (AD) applicable to McDonnell Douglas Model DC-9-80 (MD-80) and Model MD-88 series airplanes. This AD requires repetitive inspections and functional checks of the tailcone release system in accordance with McDonnell Douglas Corporation's Alert Service Bulletin A53-244, Revision 1, dated February 8, 1991. I have enclosed a copy of the AD for the Board's information. I consider the FAA's action to be completed on this safety recommendation.

Response: THE FAA AGREES WITH THIS SAFETY RECOMMENDATION. ON 1/10/91, THE FAA ISSUED AN AIRWORTHINESS DIRECTIVE (AD) (DOCKET NO. 91-NM-04-AD, AMENDMENT 39-6867) TO REQUIRE REPETITIVE INSPECTIONS AND FUNCTIONAL CHECKS OF THE DC-9 TAILCONE RELEASE SYSTEM FOR PROPER OPERATION. THE AD REQUIRES INSPECTION OF THE EMERGENCY EXIT SYSTEM IN ACCORDANCE WITH MCDONNELL DOUGLAS CORPORATION'S ALERT SERVICE BULLETINS A53-243 (EMERGENCY EXIT SYSTEM INSPECTION) AND A53-242 (TAILCONE HANDLE INSPECTION). THE FAA IS ALSO CONSIDERING THE ISSUANCE OF AN AD PROPOSING TO REQUIRE INSPECTION OF THE MD-80 TAIL CONE EMERGENCY EXIT SYSTEM IN ACCORDANCE WITH MCDONNELL DOUGLAS CORPORATION'S ALERT SERVICE BULLETIN A53-244 SIMILAR TO THAT NOTED FOR THE DC-9 AIRPLANES.
