Synopsis: ABOUT 0901 CENTRAL DAYLIGHT TIME ON AUGUST 31, 1988, DELTA AIR LINES, INC., FLIGHT 1141, CRASHED SHORTLY AFTER LIFTING OFF FROM RUNWAY 18L AT THE DALLAS-FORTH WORTH INTERNATIONAL AIRPORT (DFW), TEXAS. THE AIRPLANE, A BOEING 727-232, U.S. REGISTRY N473DA, WAS A REGULARLY SCHEDULED PASSENGER FLIGHT AND WAS EN ROUTE TO SALT LAKE CITY, UTAH.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: MODIFY THE BOEING 727 CHECKLISTS TO REQUIRE FLIGHT CREWS TO CHECK THE OPERATION OF THE TAKEOFF WARNING SYSTEM PRIOR TO EACH FLIGHT.

Response: As a result of the corrective actions taken by the issuance of AD-90-03-18, in conjunction with AD-88-22-09, Safety Recommendation A-89-127 is classified as "Closed--Acceptable Alternate Action."

Response: PLEASE SEE THE FAA'S RESPONSE. "THESE TWO AD'S WERE ISSUED TO ENSURE THAT THE TOCWS WOULD BE AVAILABLE TO PROTECT THE FLIGHTCREW FROM AN IMPROPER TAKEOFF CONFIGURATION. CONSIDERATION WAS GIVEN TO A TOCWS PREFLIGHT CHECK WHEN THE AD'S WERE BEING FORMULATED. IT WAS NOTED AT THAT TIME THAT A PREFLIGHT CHECK COULD STILL BE INADVERTENTLY OMITTED OR NOT NECESSARILY CORREECT. ADDITIONALLY, INCORPORATION OF AD 90-03-18 PREVENTS THE CHECK OF THE TOCWS WITHOUT RUNNING THE ENGINES TO A NEAR-TAKEOFF THRUST LEVEL, THUS MAKING SUCH A CHECK IMPRACTICAL."
