Synopsis: ON NOVEMBER 15, 1987, CONTINENTAL AIRLINES, INC., FLIGHT 1713, A MCDONNELL DOUGLAS DC-9-14, N626TX, WAS OPERATING AS A REGULARLY SCHEDULED, PASSENGER-CARRYING FLIGHT BETWEEN DENVER, COLORADO, AND BOISE, IDAHO. THE AIRPLANE WAS CLEARED TO TAKE OFF FOLLOWING A DELAY OF APPROXIMATELY 27 MINUTES AFTER DEICING. THE TAKEOFF ROLL WAS UNEVENTFUL, BUT FOLLOWING A RAPID ROTATION, THE AIRPLANE CRASHED OFF THE RIGHT SIDE OF RUNWAY 35 LEFT. BOTH PILOTS, 1 FLIGHT ATTENDANT, AND 25 PASSENGERS SUSTAINED FATAL INJURIES. TWO FLIGHT ATTENDANTS AND 52 PASSENGERS SURVIVED.
Recommendation: THE NTSB RECOMMENDS THAT CONTINENTAL AIRLINES, INC.: IMPLEMENT PROCEDURES TO CONDUCT SUBSTANTIVE BACKGROUND CHECKS OF PILOT APPLICANTS WHICH INCLUDE VERIFICATION OF PERSONAL FLIGHT RECORDS AND EXAMINATION OF TRAINING, PERFORMANCE, AND DISCIPLINARY RECORDS OF PREVIOUS EMPLOYERS AND FEDERAL AVIATION ADMINISTRATION SAFETY AND ENFORCEMENT RECORDS.

Response: The National Transportation Safety Board has received the Continental Airlines response signed by Mr. Joseph Corr and dated October 18, 1989, to Safety Recommendation A-88-145, concerning substantive background checks of pilot applicants. In the response, Continental Airlines specifically addresses each of the points contained in the safety recommendation. Based on this information, Safety Recommendation A-88-145 has been classified as "Closed--Acceptable Action."

Response: This is to acknowledge receipt of your letter dated September 28, 1989 requesting further clarification from Continental regarding implementation of procedures to comply with the above referenced safety recommendation. Continental's employment procedures provide for the verification of licenses and ratings maintained by the Federal Aviation Administration (FAA) and verification of disciplinary records from the FAA safety and enforcement files maintained in Oklahoma City, Oklahoma. In addition every pilot applicant is required to provide authorization for release of prior employment information. In each case, Continental requests information from prior employers on the following subjects: 1. flight hours by equipment type during the term of employment; 2. verification of positions held and length of time in each position; 3. disciplinary records; and 4. overall performance during initial and recurrent training and upgrade training. These additional steps in the investigative process ensure full compliance with Safety Recommendation A-88-145.

These checks would include verification of personal flight records and examination of training, performance, and disciplinary records from previous employers and the Federal Aviation Administration safety and enforcement records. The Safety Board notes that pilot applicants are now required to provide authorization for release of prior employment information. However, the Safety Board requests further information regarding how this authorization is used. How has Continental changed its screening of pilot applicants? Are the records of each applicant reviewed in each of the recommended areas, or is some type of random or auditing method used? The Safety Board would like Continental to clarify the meaning of gather detailed information." Response: (DUE TO LENGTH, PERTINENT INFORMATION OF THE RESPONSE HAS BEEN SUMMARIZED.) PRIOR TO THE STAPLETON ACCIDENT, CONTINENTAL WAS USING AVAILABLE PROCEDURES TO VERIFY FAA LICENSES, FAA ENFORCEMENT HISTORY, AND ACCIDENTS OR INCIDENTS INVOLVING PILOT APPLICANTS. WE HAD PROCEDURES CONSISTENT WITH INDUSTRY PRACTICE, TO VERIFY PRIOR EMPLOYMENT HISTORY. WE HAVE FURTHER REFINED OUR PROCEDURES WHEREBY APPLICANTS PROVIDE AUTHORIZATION TO GAIN RELEASE OF RECORDS FROM PRIOR EMPLOYERS. IN ADDITION, WE HAVE IMPLEMENTED ENHANCED PRE-EMPLOYMENT PROCEDURES TO GATHER DETAILED INFORMATION FROM APPLICANTS TO PROVIDE FOR A MORE THOROUGH EVALUATION OF AIRMAN SKILLS. REGARDING A-88-146, ANY SUCCESSFUL METERING PROGRAM REQUIRES KNOWLEDGE OF OTHER AIRLINE MOVEMENTS. WE HAVE ESTABLISHED A HANDS-ON, WORKING RELATIONSHIP WITH THE FAA TOWER AT STAPLETON. OUR RAMP CONTROL TOWER MAINTAINS REGULAR CONTACT WITH THE FAA TOWER AND INFORMS THEM OF HOW MANY CONTINENTAL AIRCRAFT WILL BE RELEASED WITHIN THE NEXT HOUR (OR OTHER MUTUALLY AGREED PERIOD). THE FAA TOWER THEN METERS RELEASE. AFTER DEICING IS COMPLETED, A RECORD IS MAINTAINED BY CONTINENTAL VERIFYING ELAPSED TIME BETWEEN DEICE AND TAKEOFF FOR EACH AIRCRAFT. THIS INFORMATION IS THEN USED FOR FURTHER METERING.

Response: The National Transportation Safety Board is reviewing its safety recommendation files to identify those recommendations on which there has been no correspondence for an extended time. On November 3, 1988, the Safety Board issued Safety Recommendations A-88-145 and A-88-146 to Continental Airlines. Safety Recommendation A-88-145 concerns procedures to conduct substantive background checks of pilot applicants. Safety Recommendation A-88-146 discusses company procedures to monitor ground movements of aircraft at Denver Stapleton International Airport during periods of adverse weather when deicing operations are underway, and the metering of the release of company airplanes from the deicing facility to eliminate excessive delays following deicing. Our records show that we have not received a response from your company regarding the recommended actions. We would appreciate being informed as to what efforts were made or are being made to implement this recommendation. The Safety Board's letter transmitting this recommendation is enclosed for your information. Thank you for your cooperation in this matter. We look forward to hearing from you in the near future.
