Synopsis: ON MARCH 4, 1987, FISCHER BROS. AVIATION, INC., DOING BUSINESS AS NORTHWEST AIRLINK, FLIGHT 2268, A CONSTRUCCIONES AERONAUTICAS, S.A. (CASA) C-212-CC, N160FB, CRASHED JUST INSIDE THE THRESHOLD OF RUNWAY 21R AT THE DETROIT METROPOLITAN WAYNE COUNTY AIRPORT IN ROMULUS, MICHIGAN. NINE OF THE 19 PERSONS ON BOARD WERE KILLED. THE AIRPLANE WAS DESTROYED BY IMPACT FORCES AND POSTCRASH FIRE. ON MAY 8, 1987, EXECUTIVE AIR CHARTER, INC., DOING BUSINESS AS AMERICAN EAGLE, FLIGHT 5452, A CASA C-212-CC, CRASHED SHORT OF RUNWAY 9 WHILE ON A VISUAL APPROACH TO THE AIRPORT AT MAYAGUEZ, PUERTO RICO, IN VISUAL METEOROLOGICAL CONDITIONS. BOTH CREWMEMBERS WERE KILLED AND THE FOUR PASSENGERS SUSTAINED MINOR INJURIES. THE AIRPLANE WAS DESTROYED BY IMPACT FORCES AND POSTCRASH FIRE.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: COMPLETE AS SOON AS POSSIBLE AND MAKE THE FINDINGS AVAILABLE TO THE SAFETY BOARD THE REPORT ON THE IN-HOUSE REVIEW OF THE BILATERAL AIRCRAFT TYPE CERTIFICATION PROGRAM AND THE CORRECTIVE ACTIONS TAKEN OR CONTEMPLATED AS A RESULT OF THE REVIEW.

Response: Regarding Safety Recommendation A-88-100, the Safety Board appreciates the FAA's "Review of the Construcciones Aeronauticas, S.A. CASA-212 Certification Program and the U.S. Import Type Certification Process," and the "Action Plan for Implementing Recommendations" which arose from the review. Based on the FAA implementation of the recommended improvements, Safety Recommendation A-88-100 has been classified as "Closed--Acceptable Action."

Response: Regarding Safety Recommendation A-88-100, the Safety Board looks forward to receiving the FAA's "Review of the Construcciones Aeronauticas, S.A. CASA-212 Certification Program and the U.S. Import Type Certification Process." The Safety Board has learned that the FAA is developing an "action plan" based on the report. Pending review of both the report and the action plan, the Safety Board has classified Safety Recommendation A-88-100 as "Open--Acceptable Action."

Response: IN MAY 1988, THE FAA CONDUCTED A REVIEW OF THE CIRCUMSTANCES SURROUNDING THE CERTIFICATION OF THE CASA-212. DURING THE REVIEW, EMPHASIS WAS GIVEN TO THE WORKING RELATIONSHIPS, THE IMPLEMENTATION OF THE BILATERAL AIRWORTHINESS AGREEMENT PROCEDURES, AND THE IDENTIFICATION OF AREAS WHERE IMPROVEMENTS COULD BE MADE TO ACCOMPLISH THE OBJECTIVES OF THE AIRCRAFT CERTIFICATION PROGRAM REGARDING IMPORTED PRODUCTS. THE FAA HAS PREPARED A DRAFT REPORT ENTITLED "REVIEW OF THE CONSTRUCCIONES AERONAUTICAS, S.A. CASA-212 CERTIFICATION PROGRAM AND THE U.S. IMPORT TYPE CERTIFICATION PROCESS." I WILL PROVIDE THE BOARD WITH A COPY OF THE REPORT AS SOON AS IT IS ISSUED, AND I WILL APPRISE THE BOARD OF THE FAA'S PLANNED ACTION TO IMPLEMENT THE RECOMMENDATIONS THAT ARE INCLUDED IN THE REPORT.
