Synopsis: ON OCTOBER 25, 1986, PIEDMONT AIRLINES FLIGHT 467, A BOEING 737-222, N752N, WAS A REGULARLY SCHEDULED FLIGHT OPERATING UNDER 14 CFR 121 FROM NEWARK INTERNATIONAL AIRPORT TO MYRTLE BEACH, SOUTH CAROLINA, WITH AN EN ROUTE STOP AT CHARLOTTE DOUGLAS INTERNATIONAL AIRPORT, CHARLOTTE, NORTH CAROLINA. THERE WERE 114 PASSENGERS AND 5 CREWMEMBERS ON BOARD. THE FLIGHT WAS ROUTINE UNTIL ITS ARRIVAL INTO THE CHARLOTTE AREA, WHERE INSTRUMENT METEOROLOGICAL CONDITIONS PREVAILED. AT 2004:17, THE FLIGHT WAS CLEARED FOR THE INSTRUMENT LANDING SYSTEM APPROACH (ILS) TO RUNWAY 36R. THE AIRPLANE TOUCHED DOWN AT 2007:19 AND ABOUT 2007:43 IT DEPARTED THE RUNWAY. THE AIRPLANE STRUCK THE LOCALIZER ANTENNA ARRAY LOCATED ABOUT 300 FEET FROM THE DEPARTURE END OF THE RUNWAY, STRUCK A CONCRETE CULVERT LOCATED 18 FEET BEYOND THE LOCALIZER, AND CONTINUED THROUGH A CHAIN LINK FENCE. IT CAME TO REST UPON THE EDGE OF RAILROAD TRACKS LOCATED 440 FEET FROM THE DEPARTURE END OF THE RUNWAY. THE AIRPLANE WAS DESTROYED, 3 PASSENGERS SUSTAINED SERIOUS INJURIES, AND 3 CREWMEMBERS AND 28 PASSENGERS SUSTAINED MINOR INJURIES IN THE ACCIDENT.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: ISSUE AN AIRWORTHINESS DIRECTIVE FOR A ONE-TIME INSPECTION OF THE SEAT PAN ROLLER ASSEMBLY OF THE FLIGHT ATTENDANT SEAT, TRANS AERO INDUSTRIES, PART NO. 90835, FOR EVIDENCE OF FATIGUE CRACKS.

Response: Thank you for the FAA response of February 13, 1990, to the National Transportation Safety Board's Safety Recommendation A-87-109, concerning a one-time inspection of the seat pan roller assembly of the flight attendant seat, Trans Aero Industries, part No. 90835, for evidence of fatigue cracks. The Safety Board notes that on June 22, 1989, the FAA issued an airworthiness directive (AD) (Docket No. 88-NM-149-AD; Amendment 39-6256) to require the inspections. The AD goes beyond the recommended one-time inspection by requiring reinspections every 18 months and by requiring the inspection results to be shared with the FAA. Therefore, Safety Recommendation A- 87-109 has been classified as "Closed--Acceptable Action."

Response: Thank you for your further response to Safety Recommendation A-87-109. We are pleased to note that on November 2, 1988, the FAA issued a notice of proposed rulemaking to require inspection and replacement, if necessary, of roller fittings used on flight attendant seats in certain Boeing Model 737 series airplanes. Pending your further response, this recommendation is classified as "Open-- Acceptable Action."

Response: Inquiries by the Safety Board of air carriers, Boeing Airplane Company and Trans Aero Industries disclosed that the "fleet survey" cited in your letter consisted of visual inspections of inservice and spare seats by one air carrier, and that the seat manufacturer determined there was no problem with seat roller assemblies on the basis that no rollers have been ordered by air carriers. The Safety Board is concerned that this limited action could not have determined positively that rollers are free of fatigue cracks. It is our belief that all rollers should be subject to visual and nondestructive examinations to conclusively determine that the rollers are free of fatigue cracks. We further believe that if there are rollers with fatigue cracks inservice, there is a high probability that they will fail and expose jumpseat occupants to injury in the event that minimum inertial forces cited in 14 CFR 25 for inflight and emergency landing conditions are experienced. We therefore request the FAA to reconsider the need for an airworthiness directive as called for in the safety recommendation. Safety Recommendation A-87-109 will be changed to a status of "Open--Unacceptable Action" pending your further response.

Response: As previously stated, Trans Aero Industries Part No. 90835 is a Boeing design and is similar to those currently delivered on Boeing airplanes. The seat manufacturer conducted a fleet survey to establish the nature and extent of the problem. The manufacturer found no further instances of fatigue during its survey. The FAA believes that this is an isolated incident, and does not plan to initiate rulemaking action.

Response: TRANS AERO INDUSTRIES PART NO. 90835 IS A BOEING DESIGN AND IS SIMILAR TO THOSE CURRENTLY DELIVERED ON BOEING AIRPLANES. THE SEAT MANUFACTURER IS CONDUCTING A SURVEY TO ESTABLISH THE NATURE AND EXTENT OF THE PROBLEM. THE FAA IS FOLLOWING THIS EFFORT CLOSELY AND ONCE THIS EFFORT IS COMPLETED, THE FAA WILL DETERMINE ITS COURSE OF ACTION. I WILL KEEP THE BOARD APPRISED OF THE FAA'S PROGRESS ON THIS RECOMMENDATION.
