Synopsis: ON JANUARY 25, 1982, SUNDSTRAND DATA CONTROL, INC., ISSUED SERVICE BULLETIN NO. 012-0296-109, "COMMUNICATIONS COCKPIT VOICE RECORDERS/MODELS AV-557A, AV-557B, AND AV-557C/TAPE DECK HOUSING CONNECTOR CHECK." THE SERVICE BULLETIN STATED: "IT HAS BEEN DETERMINED THAT SOME CVRs (COCKPIT VOICE RECORDERS) WERE MANUFACTURED WITH CONNECTORS THAT CONTAIN SPLIT-RING, RATHER THAN FULL-RING SOCKETS. USING CONNECTORS WITH SPLIT-RING SOCKETS MAY CAUSE CVRs TO OPERATE INTERMITTENTLY, GIVE ERRONEOUS SELF TEST INDICATIONS, AND CONTRIBUTE TO SENSOR OR TAPE-OFF REEL PROBLEMS." THE SERVICE BULLETIN INSTRUCTED OWNERS OF THE CVRs HOW TO EXAMINE THEM FOR THE TYPE OF CONNECTOR SOCKET USED, AND INSTRUCTED THEM TO CONTACT THE FACTORY FOR REPLACEMENT OF THE SPLIT-RING SOCKETS. THESE TYPE CVRs ARE INSTALLED ON SUCH AIRCRAFT AS THE BOEING 727, 737, 747, AND THE DOUGLAS DC-9 AND DC-10.
Recommendation: SINCE A CVR WILL NOT OPERATE PROPERLY WITH A SPLIT-RING CONNECTOR AND BECAUSE OF THE IMMENSE VALUE OF THE CVR RECORD IN AIRCRAFT ACCIDENT INVESTIGATIONS, THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: ISSUE AN AIRWORTHINESS DIRECTIVE (AD) MAKING MANDATORY AN INSPECTION (AS SOON AS PRACTICAL DEPENDING ON AVAIL ABILITY OF SPARE UNITS AND THE CAPABILITY TO REPLACE THE DEFECTIVE UNITS) FOR INCORRECT SPLIT-RING SOCKETS SPECIFIED IN SUNDSTRAND DATA CONTROL SERVICE BULLETIN DOCUMENT NO. 012-0296-109, DATED JANUARY 25, 1982, TITLED "COMMUNICATION -- COCKPIT VOICE RECORDERS/MODELS AV-557A, AV-557B, AND AV-557C/TAPE DECK HOUSING CONNECTOR CHECK," AND REQUIRE REPLACEMENT OF INCORRECT CONNECTORS AT THE EARLIEST POSSIBLE DATE.

Response: Airworthiness Directive 83-16-01, issued by the FAA on July 28, 1983, requires an inspection for and replacement of incorrect split-ring socket connectors in accordance with Sundstrand Data Control Service Bulletin Document No. 012-0296-109, dated January 25, 1982. This AD fulfills the intent of the Safety Board's recommendation. Therefore, Safety Recommendation A-82-43 has been placed in a "Closed--Acceptable Action" status.

Response: FAA LTR: THE FAA'S TRANSPORT AIRPLANE CERTIFICATION DIRECTORATE HAS ASSESSED THE PROBLEMS CITED IN SAFETY RECOMMENDATION A-82-43 AND THE NECESSITY FOR ISSUANCE OF AN AD. WE EXPECT TO HAVE THE FINAL RULE ISSUED BY MID-JANUARY 1983. WE WILL ADVISE THE BOARD WHEN THIS FINAL RULE IS ISSUED.

Response: We are pleased to be informed that the FAA agrees that mandatory action is warranted and that a Notice of Proposed Rulemaking (NPRM) to issue an AD will be published shortly. However, we request that final action be completed by January 30, 1983, rather than July 30, 1983, as proposed in your July 19, 1982, letter. Pending receipt of the AD, this recommendation is classified in an "Open--Acceptable Action" status. In a related matter to the connector problems in the cockpit voice recorder, the Safety Board received, on July 23, 1982, a Service Information Letter (S.I.L. No. 216) from Sundstrand Data Control, Inc., dated June 18, 1982, (enclosed) which said that certain connectors within the Sundstrand digital flight data recorder (DFDR) should be inspected and replaced because these may "result in an intermittent electrical contact which could cause erratic operation or even stopping of the recorder." Our technical staff informed your engineers of this letter on July 29, 1982. It is Data Source: NTSB Recommendations to FAA and FAA Responses our understanding that an NPRM to issue an AD on this item will also be forthcoming. The Safety Board fully endorses the issuance of an AD to correct this problem with the Sundstrand DFDR.

Response: FAA LTR: THE FEDERAL AVIATION ADMINISTRATION HAS COMPLETED ITS ASSESSMENT OF THE PROBLEMS CITED IN SAFETY RECOMMENDATION A-82-43 AND AGREES THAT MANDATORY ACTION IS WARRANTED. A NOTICE OF PROPOSED RULEMAKING (NPRM) TO ISSUE AN AIRWORTHINESS DIRECTIVE IS BEING INITIATED BY THE TRANSPORT AIRPLANE CERTIFICATION DIRECTORATE IN THE NORTHWEST MOUNTAIN REGION. WE EXPECT THAT THE NPRM WILL BE ISSUED SHORTLY, AND THAT FINAL ACTION WILL BE COMPLETED BY JULY 30, 1983.
