Synopsis: ON APRIL 8, 1981, EASTERN AIRLINES FLIGHT 60 FROM NEW ORLEANS, LOUISIANA, TO NEW YORK MADE AN EMERGENCY, GEAR-RETRACTED LANDING ON RUNWAY 22R AT JOHN F. KENNEDY (JFK) INTERNATIONAL AIRPORT, JAMAICA, NEW YORK. THE LANDING OF THE BOEING 727 WAS FOLLOWED BY AN EMERGENCY EVACUATION OF THE AIRCRAFT. ALL 67 PASSENGERS AND 6 CREWMEMBERS WERE EVACUATED WITHOUT INJURY.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: REQUIRE THE REVISION OF THE ABNORMAL PROCEDURES SECTION OF BOEING 727 AIR CARRIER OPERATOR FLIGHT MANUALS, AS NEEDED, REGARDING THE LANDING GEAR UNSAFE INDICATION, TO INCLUDE ADDITIONAL INFORMATION RELEVANT TO THE GEAR POSITION INDICATOR LIGHTS AND THE LANDING GEAR WARNING HORN SYSTEM, AND THE FACT THAT THEY ARE NOT INDEPENDENT AND ARE NOT REDUNDANT LANDING GEAR POSITION INDICATING SYSTEMS.

Response: We have examined the revisions to the Boeing 727 Operations Manual which were issued on April 1, 1982. We have also noted the contents and intent of the FAA's memorandum dated July 19, 1982, to all Regional Flight Standards Division Managers. The actions taken satisfy Safety Recommendations A-81-97 and -98 which we have now classified in a "Closed--Acceptable Action" status.

Response: FAA LTR: ON APRIL 1, 1982, IN RESPONSE TO THE BOARD'S RECOMMENDATIONS, THE BOEING AIRCRAFT COMPANY PUBLISHED A REVISION TO THE BOEING 727 OPERATOR FLIGHT MANUAL. THE REVISION CONTAINS CREW INSTRUCTIONS AND ILLUSTRATIONS ON THE USE OF THE LANDING GEAR POSITION INDICATOR VIEWING PORTS. ADDITIONALLY, THE REVISION CONTAINS TRAINING MATERIALS ON THE SAME SUBJECT. TO ENSURE THAT THE REVISION IS INCORPORATED IN THE OPERATION MANUALS OF AIR CARRIERS OPERATING BOEING 727 AIRPLANES, WE ISSUED FORMAL INSTRUCTIONS TO ALL REGIONAL FLIGHT STANDARDS DIVISION MANAGERS. THE INSTRUCTIONS REQUESTED THAT ALL CONCERNED PRINCIPAL OPERATIONS INSPECTORS BE ADVISED OF THE SAFETY BOARD'S RECOMMEN DATIONS AND ISSUANCE OF THE REVISION TO BOEING 727 AIR CARRIER OPERATOR FLIGHT MANUALS. THE INSTRUCTIONS ALSO DIRECTED THEIR ATTENTION TO THE FLIGHT CREW TRAINING IMPLICATIONS ASSOCIATED WITH THE REVISION AND EMPHASIZED THE NEED TO ENSURE THAT THEIR RESPECTIVE AIR CARRIER'S TRAINING PROGRAMS REFLECT THE ADDED INFORMATION.

Response: We have reviewed the Boeing 727 Operations Manual revisions regarding landing gear position indication and system operation cited in your response. The technical content of these revisions, dated April 1, 1982, is in full accord with the intent of our recommendations. Your reply indicating that Principal Operations Inspectors (POI) of fleets using a B-727 aircraft will be notified of the revision and will take appropriate measures to assure incorporation of this information in training programs is positive; however, there is no indication in the response that FAA will require the Boeing Operations Manual revisions to be incorporated into the flight manuals of each air carrier operator using B-727 aircraft. Accordingly, these recommendations are classified "Open--Acceptable Action" pending clarification of your intent regarding requirements for air carrier operator flight manual revisions.

Response: FAA LTR: THE FAA CONCURS IN THE INTENT OF THESE RECOMMENDATIONS. WE HAVE RECENTLY CONTACTED BOEING FLIGHTCREW TRAINING OFFICIALS AND THE FOLLOWING INFORMATION WAS FORTH COMING FROM OUR DISCUSSIONS. BOEING IS PRESENTLY COMPLETING A BLOCK REVISION OF B-727 OPERATIONS MANUALS. INCLUDED IN THIS REVISION WILL BE CREW INSTRUCTIONS AND ILLUSTRATIONS CONCERNING USE OF LANDING GEAR POSITION INDICATOR VIEWING PORTS SHOWING AT LEAST "LOCKED" AND "UNLOCKED" INDICATIONS. ALSO INCLUDED IN THE BLOCK REVISION WILL BE THE TRAINING MATERIAL RELATIVE TO THE ABNORMAL PROCEDURES SECTION OF BOEING 727 AIR CARRIER OPERATOR FLIGHT MANUALS. AFTER PRINTING, THESE REVISIONS WILL BE SENT TO ALL KNOWN OPERATORS OF THE B-727. THE POI'S WILL ALERT THE OPERATORS TO THE PUBLICATION OF THIS REVISION, AND APPROPRIATE MEASURES WILL BE TAKEN TO INSURE THAT TRAINING MATERIALS ARE INCORPORATED IN FUTURE FLIGHTCREW TRAINING PROGRAMS.
