Details
Synopsis: ON SEPTEMBER 6, 1977, ALASKA AERONAUTICAL INDUSTRIES FLIGHT 30, A DHC-6-200, CRASHED INTO MOUNT ILIAMNA WHEN THE AIRCRAFT STRAYED OFF COURSE EN ROUTE FROM ILIAMNA, ALASKA, TO ANCHORAGE. THE 11 PASSENGERS AND 2 CREWMEMBERS DIED IN THE ACCIDENT. THE NATIONAL TRANSPORTATION SAFETY BOARD'S INVESTIGATION REVEALED POOR OPERATIONAL PRACTICES, POOR MAINTENANCE PRACTICES, AND INADEQUATE TRAINING PRACTICES BY THE OPERATOR, AND INADEQUATE SURVEILLANCE OF THE OPERATOR BY THE FEDERAL AVIATION ADMINISTRATION. ALASKA AERONAUTICAL INDUSTRIES IS THE 12TH LARGEST COMMUTER AIRLINE IN THE NATION AND TRANSPORTS MORE THAN 150,000 PASSENGERS EACH YEAR. AS SUCH, THE SAFETY BOARD BELIEVES THAT THE COMPANY'S OPERATING PROCEDURES MUST PROVIDE A HIGH LEVEL OF SAFETY TO THE PUBLIC, AND THAT FAA'S SURVEILLANCE MUST INSURE THAT ADEQUATE STANDARDS ARE MAINTAINED.
Recommendation: THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: REVIEW THE MAINTENANCE PROCEDURES OF AIR TAXI AND COMMUTER AIRLINES OPERATORS TO EVALUATE THE EFFECTIVENESS OF THOSE PROCEDURES AND TO INSURE ADEQUATE COMPANY CONTROL.

Response: We have examined the revised far part 135, subpart j, relating to maintenance, preventive maintenance and alterations. We are satisfied that the upgraded maintenance requirements and procedures fulfill this recommendation, which is also now classified as closed--acceptable action.

Response: From the accident report of Antilles Air Boats, Inc. Grumman G21A, N7777V St. Thomas Virgin Islands on September 2, 1978. The Board issued this report on 6/28/1979. On May 17, 1978, the Safety Board issued Safety Recommendations A-78-37 through -41 in connection with a commuter airlines accident which occurred on September 6, 1977. The recommendations are applicable to this accident investigation; thus, the Safety Board reiterates that them to the Federal Aviation Administration.

THE PROPOSED UPDATE (NPRM 77-17) OF FAR PART 135 WILL, WHEN ADOPTED, REQUIRE ADDITIONAL MAINTENANCE CONTROLS BEYOND THOSE PRESENTLY IN EFFECT. 12/29/78 - FAA LTR: SECTIONS 135.411 THROUGH 135.443 CONTAIN THE AIRCRAFT MAINTENANCE REQUIREMENTS FOR FAR PART 135 OPERATIONS.
