Details
Synopsis: A PAN AMERICAN BOEING 747 STRUCK THE APPROACH LIGHT STRUCTURE (ALS) FOR RUNWAY 19L WHILE TAKING OFF FROM RUNWAY 01R AT THE SAN FRANCISCO INTERNATIONAL AIRPORT. THE CREW CONTINUED THE TAKEOFF, AND AFTER AN INFLIGHT INSPECTION FOR DAMAGE, DUMPED FUEL AND RETURNED FOR A LANDING AT SAN FRANCISCO. TWO PASSENGERS WERE INJURED DURING THE IMPACT WITH THE ALS AND EIGHT OTHERS SUSTAINED SERIOUS BACK INJURIES DURING THE EVACUATION AFTER THE LANDING.
Recommendation: THE FAA REVIEW THE CRITERIA APPLIED TO THE INSTALLATION OF THESE PANELS AND EFFECT WHATEVER ACTION IS APPROPRIATE TO IMPROVE THE INSTALLATION SO THAT THE PANELS WILL STAY IN POSITION DURING SURVIVABLE IMPACT LOAD IMPOSED ON THE CABIN STRUCTURE.

Response: THEY ARE PRESENTLY REVIEWING WITH THE MANUFACTURER THE DESIGN ASPECTS THAT CONCERN THE RETENTION OF THE CEILING PANELS AS WELL AS INSTRUCTIONS FOR THEIR INSTALLATION AND REMOVAL. EXPECT IN NEAR FUTURE TO HAVE COMPLETE RESOLUTION OF POINTS IN RECOMMENDATION.
