Details
Synopsis: A PAN AMERICAN BOEING 747 STRUCK THE APPROACH LIGHT STRUCTURE (ALS) FOR RUNWAY 19L WHILE TAKING OFF FROM RUNWAY 01R AT THE SAN FRANCISCO INTERNATIONAL AIRPORT. THE CREW CONTINUED THE TAKEOFF AND, AFTER AN INFLIGHT INSPECTION FOR DAMAGE, DUMPED FUEL AND RETURNED FOR A LANDING AT SAN FRANCISCO. TWO PASSENGERS WERE INJURED DURING THE IMPACT WITH THE ALS AND EIGHT OTHERS SUSTAINED SERIOUS BACK INJURIES DURING THE EVACUATION AFTER THE LANDING. THE AIRCRAFT HAD BEEN DISPATCHED FOR A DEPARTURE FROM A CLOSED RUNWAY AND, UPON CHANGING TO AN OPEN RUNWAY, THE CREW DID NOT RECOMPUTE THE PROPER REFERENCE SPEEDS FOR TAKEOFF UNDER THE EXISTING CONDITIONS.
Recommendation: REVIEW THE PROCEDURES FOR THE ISSUANCE OF NOTAM AND AIRAD FOR STANDARDIZED IMPLEMENTATION WITHIN THE APPROPRIATE FAA FACILITIES AND MODIFY THE PROCEDURES TO ASSURE THAT INFORMATION PERTINENT TO "SAFETY OF FLIGHT" IS DISSEMINATED WITHOUT DELAY.

Response: As a result of this review and evaluation, the faa issued handbook 7930.2, national notice of airmen (notam) system, effective may 1,1977, which prescribes the faa's notam system procedures and provides guidance for all personnel charged with the origination, the faa issued ac no. 210-4, effective concurrent with order 7930.2 on may 1, 1977, and reflecting the contents of the order. The term airad was replaced by notam (l) which is included in the order and advisory circular. The faa believes it has satisfactorily fulfilled the intent of recommendation a-72-1 and plans no further action.

A MANUAL WHICH CONSOLIDATES AND STANDARDIZES ALL INFORMATION CONCERNED WITH NOTICES TO AIRMEN WILL BE DEVELOPED. COMMENTS FROM OUTSIDE THE AGENCY WILL BE SOLICITED PRIOR TO PUBLICATION. 8/2/72. NOTAM STUDY COMPLETED. A LETTER OF EXPLANATION OF THE PROCEDURES TO STANDARDIZE IMPLEMENTATION SENT TO THE APPROPRIATE FAA FACILITIES. A COPY OF THIS LETTER WILL BE FORWARDED TO THE BOARD. 6/20/77 - FURTHER RESPONSE FROM FAA STATING: THE FAA INITIATED A STUDY TO REEVALUATE THE NOTAM SYSTEM. AS A RESULT OF THIS REVIEW AND EVALUATION, THE FAA ISSUED HANDBOOK 7930.2, NATIONAL NOTICE TO AIRMEN (NOTAM) SYSTEM, EFFECTIVE MAY 1, 1977, WHICH PRESCRIBES THE FAA'S NOTAM SYSTEM PROCEDURES AND PROVIDES GUIDANCE FOR ALL PERSONNEL CHARGED WITH THE ORIGINATION, ISSUANCE, AND DISSEMINATION OF NOTICE TO AIRMEN DATA.
