NTSB Identification: MIA95WA017
On November 2, 1994, about 0725 Atlantic standard time, N5860Y, a Piper PA-23-250, operated by Congo Air Charter Company, as a 14 CFR Part 129, on demand, domestic, passenger flight, crashed after takeoff from Mangrove Key, Bahamas. Visual meteorological conditions prevailed at the time and a VFR flight plan was filed for the proposed flight to Nassau, Bahamas. The airplane was substantially damaged and the pilot and the one passenger received fatal injuries. The flight was originating at the time of the accident.

Company personnel reported the flight crashed into the ocean immediately after departure and floated for a short time before sinking in about 15 feet of water.
