On May 9, 2008. about 0645 UTC, N5014V, Bell 206 L-1, operated by Petroleum Helicopters Inc., collided with water near Muanda, Democratic Republic of Congo (DRC). The helicopter was repositioning between tanker ships when the collision occurred. The pilot, the sole occupant, was fatally injured.
