On October 11, 2001, at 2333 Central Daylight Time, a Fairchild Swearingen SA-226-TC Metro, Canadian registration C-GYPA, operated by Perimeter Airlines, Ltd, as an air ambulance flight, crashed during a missed approach to runway 01 at the airport near the remote village of Shamattaw located in the Canadian province of Manitoba. The airplane was destroyed and there was a ground fire. Both pilots were killed. The third occupant, a flight nurse, was seriously injured. The ceiling was reported as 2,500 feet overcast by witnesses at the time of the accident. Dark night conditions prevailed. An instrument flight plan had been filed for the flight. A cockpit voice recorder was recovered from the accident site. The investigation is being conducted under the jurisdiction of the government of Canada.
